I gave my doctor a dose of MY OWN “cholesterol clarity” by having an NMR done. When the book comes out, I’m ordering a copy for her–she obviously needs ongoing therapy. 🙂

Hi Jimmy — good interview. I have very similar beliefs about the aetiology of CVD. I have to make some criticisms of Lundell’s details, though. Not sure whether he was intentionally oversimplifying (to more concisely make his primary points) or not, but here goes …..
I have studied M. Brownlee’s work, which Lundell mentions. Brownlee, it could be reasonably argued, has proven the cause of diabetic complications in the MICROvasculature — this work was the subject of the Banting lecture alluded to. In the same lecture Brownlee lays out an interesting (and independent) hypothesis for the cause of CVD in the MACROvasculature, while fully recognizing that this is consistently associated with INSULIN RESISTANCE but NOT with HYPERGLYCEMIA (and the correlation is also much weaker than that between microvascular disease and hyperglycemia).
The evidence is overwhelming that hyperglycemia is NOT causal in CVD such as coronary artery atheroschlerosis. Statistical evidence, however, IS consistent with IR playing a causal role.
Brownlee’s hypothesis attempts to explain how chronic IR could be causal, but by no means has strong evidence behind it. I find it interesting, though, because the unique direct exposure of the macrovasculature endothelium to constant guaranteed supplies of glucose as fuel somewhat buttresses the theory from an evolutionary point of view (an argument that Brownlee himself does not make, to my knowledge).
Nevertheless I agree with the focus by Lundell on postprandial serum glucose as being key, since this is actually much more a function of whole-body IR than of, say for instance, diabetogenous degradation of glucose-stimulated endogenous insulin secretion and fasting glucose (these two are strongly related). And there is a large amount of evidence from the statistics on diabetics and pre-diabetics to support this idea. Just one important example that is illustrative of the contrast between fasting/basal hyperglycemia and postprandial hyperglycemia is the complete lack of an increase in CVD risk for isolated-IFG and the clearcut increase for isolated-IGT and the very strong increase for CGI (these are the most heavily insulin resistant type of pre-diabetics with the highest postprandial BG peaks, whereas i-IFG types have nominally no IR whatsoever).
I tend to agree with Lundell that injury to the endothelial cells (due to the chronic stresses induced by modern diet, primarily) are the starting point — many researchers would likely argue that the injury to the cells comes from oxidized lipoproteins, etc. (so that these come first), but I agree with Lundell.
Brownlee offers one theory for how such injury might occur in the macrovasculature — probably the best one that I know of but still just a hypothesis. Hyperglycemia does NOT cause the initial injury in the macrovasculature, as it does in the microvasculature. There is still a LOT of basic research left to be done regarding the aetiology of CVD in the macrovasculature, IMO.
I just listened to the podcast and I must speak up for my fellow nursing professionals. I can see from Dr. Lundell’s photograph that he is an older gentleman and he likely thinks of those of us in the nursing profession as little more than handmaidens to the physician. His dismissive comments that under the ACA more people will be seeing ‘ancillary health people’ instead of (real) doctors seems to imply that those people being seen by Nurse Practitioners will be getting substandard care. Nurse Practitioners are highly trained health care professionals and are a valuable asset to the medical community. There are many situations where patients can be well served by the Nurse Practitioner. All Nurse Practitioners operate under the supervision of a physician. I’m sure Dr. Lundell is very knowledgeable about heart disease, but he needs to come into the 21st Century when it comes to the roles of health care professionals.

Jimmy …..absolutely fantastic episode. It’s about the sugar and not the fat. How radical to start getting away from blindly tracking Total cholesterol. Not sure I’m fully ready for the do-it-yourself era of medicine yet but here goes. Thanks Dr Lundell for your unfiltered candor. You are both doing a terrific service for those of us looking for truth.
